A Parent's Two-Week Observation Checklist

A simple structure for recording your child's breathing, sleep and nasal symptoms before an evaluation.

Bringing structured observations to a consultation makes the evaluation more useful. Record what you see for two weeks, without trying to interpret it.

What to record daily

  • · Was the mouth open at rest during the day?
  • · Was there snoring at night? How loud?
  • · How many times did the child wake?
  • · Was the nose blocked? Which side?
  • · Any cough, sneezing or ear discomfort?
  • · Was school attended?
